Before you begin, please make sure you have the names, addresses and
phone numbers of your children's doctors, the names and phone numbers of two emergency
contacts and the name and policy number of your children's medical insurance carrier.
Also, if you plan to pay by credit card, please have your credit card ready.
Also note: Once you start the registration,
you can't stop and then pick up later. If you discontinue registration,
you must start over from the beginning. Registration will take 10-15 minutes to complete.
You must also have a printer. During registration, you must be able to
print medical exam forms for the children you register, as well as
a voucher.
- If you will pay by check, registration is not complete until SPAC receives the
printed voucher and your check for any fees. The voucher must be
received within seven days (i.e., by 5/19/2008)
of this online registration.
- If you pay by credit card, registration is complete
if your credit card charge is processed successfully. Please print
and save (for your records) the confirmation that is produced when paying by credit card.
You have until 9/7/2008
to submit the completed exam form from your child's doctor if one is required.
